Form for Distance Learning Students
to request a TexShare Card

College faculty and students enrolled in distance learning or traditional classes* can use this form to request a TexShare library card. The TexShare card will allow the cardholder to check out books at participating Texas college, university, and public libraries . You must first have a Lone Star College library card to request a TexShare Card.

*If you attend any classes on campus at Lone Star College-North Harris, Lone Star College-Tomball, or the Willow Chase Center, you are asked to get a picture student ID to use as your library card. After obtaining your ID/library card, you may request a TexShare library card using this form.

Your TexShare card will be available in a few days and can be picked up at your preferred location or mailed to you.

	TexShare Card Borrower Responsibilities

1. Appropriate identification as required by the lending library must be presented.

2. Materials must be returned BY THE BORROWER TO THE LENDING LIBRARY by the due date shown on the item either in person or by insured mail

3. Materials must be returned immediately if recalled by the lending library.

4. Borrowers are responsible for payment of any fines, damage costs, replacement cost of lost materials, or any other charges assessed by the lending library. Holds will be placed on Lone Star College System records until such charges are cleared.

5. Borrowers agree to surrender the TexShare Card to library staff if requested.
